Recently, our clinic was approached by patients who went to other clinics to receive the breast augmentation fat grafting surgery for consultation.
Their main problem is regarding the lumps that had formed n their breasts after the surgery. There are different cases: some claimed that there is a lump that is greater than 4cm in diameter and some said that there are a few lumps (smaller than 4cm in diameter) in one breast. According to them, from the looks the breast seems completely normal but when they touch their breasts, the feeling of the lumps makes them feel uncomfortable. To some, it is a big problem as they fear that it might be a symptom of breast cancer.

Fat grafting for breast augmentation has grown gradually acceptable in the cosmetology market. This is mainly due to the breakthroughs that were made in terms of the surgery throughout the recent 10 years. Compared to breast implant or breast augmentation prosthesis, the grafted fat used for breast augmentation has more pros than cons. For example, ruptures that can happen on breast implants can be avoided. Besides, there is also no risk of penetration of substances into the breasts. In terms of appearance, the breast looks perfectly natural and patients will not exposed to post-operation capsular contracture. However, there are a few disadvantages of breast augmentation fat grafting, such as the survival rate of the breasts is not stable, patients are prone to breast cancer, breast calcification, etc.

Despite being a disadvantage, the survival rate of breast can actually be increased through the technology of stem cells. It must be known that the risk of getting breast cancer does not have anything to do with autologous fat grafting. This can be scientifically proven. Hence, the other major problems that patient should keep an eye on is the post-operation calcification and the formation of lumps. According to medical journals, the success rate of autologous fat grafting is largely related to the process of injection. To avoid fat necrosis and lumps after injection, the few tips below must be stressed on:
  • The process of extracting fat/lipids must be strictly conducted under a sterile and aseptic condition. If the fat extracted is contaminated, the percentage of fat necrosis after operation is increased.
  • During the purification of the fat, always be sure to avoid destructing the fat. Lipids are the same as our somatic cells. Too much work or process will damage the lipids’ structurally and chemically.
  • It is necessary to carry out the injection by different levels. Through this, patients will have more blood supply is available to provide nutrients after the fat is injected.
  • The injected fat must be in contact with related tissues. In order to survive, the fat to be injected depends on the health tissues’ nutrients hence the first two days after injection are very important. Every time an injection is conducted, the fat must always be made sure to be in contact with the tissues in the transplantation area. The ‘hollow’ feeling must also be avoided because this feeling represents that the syringe needle does not penetrate into the original fat; the injected fat is not in contact with the original fat.
  • After injection, if the breasts are softly elastic, it usually tells that the operation is successful. In contrary, if the breasts feel like solid or hard, this resembles that the risk of fat necrosis is higher. The breast will become softer over time if the grafted fat has stabilized and survived. If too much fat is injected into the same position, a lump will form and the inner fat will be damaged due to malnourishment. In one to two months it will worsen into a hard lump or cystis.

Upon realizing the formation of lumps or cystis, it is focal to tend to the problem as early as possible. This is the best as when it isn’t taken care for a long time it will become a fibrosis which brings more complications.
Normally, lumps that are smaller than 2cm in diameter can be take care with fomentation and massage. Consequently, the lump will derease in size and disappear. For lumps that are greater than 2cm in diameter, the lumps won’t go away if nothing is done. Even though this is not breast cancer and it does not increase the risk of breast cancer, to those who are facing the problem, the surgeons who gad carried out the operation should take up the responsibility to resolve the problem instead of treating it with negligence. The most important thing is to pay attention to the way the injection is carried out during the surgery.

In a nutshell is better than cure. A professional and authorized way of injection is the best means to avoid any unnecessary post-operation problems like fat necrosis and cystis.
